各位師生同仁大家好:

    學務處衛保組為鼓勵您做好自我健康把關，提供您在校健檢之

便捷服務，將辦理「106學年度上學期新生複檢」暨

「教職員工生眷優惠健檢」活動，誠摯歡迎您一起加入

「自主健康管理」的行列。

活動內容如下：

1. 活動時間：106年11月14日(二)上午9:00~下午1:30

2. 活動地點：風雲樓3F國際學生活動中心

3. 承辦醫院：106學年新生健檢廠商-怡仁醫院

4. 活動內容：請詳閱附加檔案

活動說明:

1. 106學年度上學期新生複檢：請今年新生攜帶健檢報告到現場進行複檢。

   如未達免費複檢標準可「自費」優惠複檢。

2. 教職員工生眷優惠健檢：於活動當天至現場進行繳費健檢

   (要注意有些項目必須空腹6-8小時)

備 註：

1. 有關衛生署「B肝表面抗體陰性」建議，請參閱附件。

2. 活動當天B型肝炎疫苗限量供應，需施打B肝疫苗者

   ，請至附件閱讀詳情。

Dear all:

     In order to encourage you to be aware of your health

condition, Division of Health Service will provide a Health

Examination on campus. We will hold 「106 School year 1st semester

New Students Health Examination Recheck」 and 「Staff, Students

and Family Member Health Check Promotion」these two events,

we sincerely hope that you can participate in our

「Self-Health Assessment」group.

Event information is listed below:

1. Time: 2017 Nov 14th (Tue) 9:00 am to 1:30 pm

2. Venue: Feng-Yun Building 3rd floor,

   International Students Activity Center.

3. Hospital: Yee Zen Hospital.

4. Event information: Please read the attached file.

Event description:

1. 106 School year 1st semester New Students Health Examination

   Recheck: Please bring your Health Examination Report to do

   the recheck. If you have not reached the criteria for a free

   health exam, you can do the examination at your own expense.

2. Staff, Students and Family Members Health Check Promotion:

   Please come to our event and do the examination

   at your own expense.

   (Please note that some tests require you to

   fast for 6-8 hours).

Note:

1. For advises from Ministry of Health and Welfare regarding

   those that have negative Hepatitis B surface antibody,

   please read the attached file.

2. Those who wish to take the Hepatitis B vaccine,

   please read the attached file.
