1. 依據學校衛生法及傳染病防治法之規範辦理。

2. 108年9月4日及5日，假本校校友體育館統一辦理108學年度第一學期新生健檢活動。
   2019/9/4 and 9/5, New Students Health Examination will be held at the Alumni Gymnasium.
    
3. 本年度健檢承辦醫院為桃園縣怡仁綜合醫院。
   This year’s cooperated Hospital is Taoyuan Yeezen General Hospital.

5. 請攜帶本學年度健檢費用$730元，並於健檢當天空腹6-8小時到健檢現場。
   Please bring the examination fee NTD 730, also please fast for 6~8 hours prior to the exam.
    
6. 本校接受之有效健檢報告為108年6月1日以後健檢之報告。健檢報告項目必須包含本校學生健康檢查資料卡所載項目，缺少者必須補齊後始受理註冊。
   The school accepts checkup reports done after 2018/6/1. The report must include all items listed in the NTHU New Students Health Examination Card, items lack must be completed then only can the registration process proceed.
    
7. 註冊當日無法完成健檢者，除自行負責完成健檢外，對未完成健檢導致無法完成註冊程序之後果仍需自行負責。
   Students who cannot complete the medical checkup on Registration Day, will be held full responsibility if unable to complete the registration process.
