  • 提醒同學Notice for students：
  1. 如因故至上述合作醫院健檢者，請攜帶「合作醫院繳費收據」， 至健檢現場-校友體育館，找本組人員蓋註冊程序章。
    If you go to the cooperated hospitals listed above to do the checkup, please bring your receipt to the Health Examination site on Registration Day, Alumni Gymnasium, and find the staffs of our division to stamp your Registration Procedure Sheet.
  2. 在其他醫院健檢者，請持本校學生健檢資料卡至地區級(含)以上醫院進行健檢， 並繳交已完成之學生健康檢查資料卡報告正本至本組，始辦理註冊蓋章手續。
    Checkup done in other hospitals, please bring NTHU Health Examination Card to the hospital (Hospital class District Hospital and above) to do the checkup, then submit the genuine report to our division to complete registration procedure.
  3. 因應各醫院健檢報告作業時間，請務必於註冊日前10-14日完成健檢作業。
    Every hospital needs 10-14 working days to complete the medical checkup，so please do your medical checkup earlier to avoid any inconvenience on Registration day.
