NTHU 104 School year 1^st semester New Students Health Examination Recheck and Staff, Students and Family Member Health Check Promotion

    學務處衛保組為鼓勵您做好自我健康把關，提供您在校健檢之便捷服務，將辦理「104學年度上學期新生複檢」暨「教職員工生眷優惠健檢」活動，誠摯歡迎您一起加入「自主健康管理」的行列。活動內容如下：

活動時間：104年11月04日(三)上午9:00~下午13:30
活動地點：鏡廳

承辦醫院：104學年新生體檢廠商-怡仁醫院
活動內容：請詳閱附加檔案

活動說明:

1. 104學年度上學期新生複檢：請今年新生攜帶體檢報告到現場進行複檢。如未達免費複檢標準可 自費 優惠複檢。
2. 教職員工生眷優惠健檢：於活動當天至現場進行繳費健檢(要注意有些項目必須空腹6-8小時)

備 註：

1. 有關衛生署「B肝表面抗體陰性」建議，請參閱附件。
2. 需施打B肝疫苗者，請至附件閱讀詳情及完成報名登錄。
3. B型肝炎疫苗線上報名：請上校務資訊系統->校內其他系統->健康照護系統->線上預約->活動報名 。
4. 校內眷屬預報名本次活動，請來電衛保組03-5731051報名。

    In order to encourage you to be aware of your health condition, Division of Health Service will provide a Health Examination on campus. We will hold 「104 School year 1^st semester New Students Health Examination Recheck」 and 「Staff, Students and Family Member Health Check Promotion」these two events, we sincerely hope that you can participate in our 「Self-Health Assessment」group. Event information is listed below:

Time: 2015 November 4^th (Wed) 9:00 am to 13:30 pm

Venue: Auditorium

Hospital: Yee Zen Hospital

Event information: Please read the attached file

Event description:

1. 104 School year 1^st semester New Students Health Examination Recheck: Please bring your Health Examination Report to do the recheck. If you have not reached the criteria for a free health exam, you can do the examination on your own expense.
2. Staff, Students and Family Members Health Check Promotion: Please come to our event and do the examination on your own expense. (Please note that some tests require you to fast for 6-8 hours).

Note:

1. For advises from Ministry of Health and Welfare regarding those that have negative Hepatitis B surface antibody, please read the attached file.
2. Those who wish to take the Hepatitis B vaccine please read the attached file and register online.
3. Hepatitis B vaccination online registration: Please log in to the Academic Information Systemà->Other Services in campusà->Health Careà->Reservationà->Event Registration
4. Family members, who wish to participate in this event, please call Division of Health Services 03-5731051 to register.
