      又到了流感疫苗接種的季節，為了方便清大校內教職員工生及眷屬

可直接於校內施打流感疫苗，本組特協調怡仁綜合醫院，同意提供

自費季節性流感疫苗及免費到校施打之服務。

(報名人數未達50人，則取消活動；本活動不開放12歲以下兒童施打)

一、 報名期間: 104年10月12日至11月02日17:00。

二、 活動時間：104年11月04日(三) 09:00-13:30。

三、 活動地點：鏡廳

四、 活動費用：季節性流感疫苗（巴斯德）需自費450元 (現場繳費)。

五、 流感疫苗線上報名：

     請上校務資訊系統 ->校內其他系統-> 健康照護系統->線上預約->活動報名。

     (如是眷屬，請來電至衛保組 03-5731051 將協助報名)。

六、 如報名後因故取消，務必事先來電或來信 clinic@my.nthu.edu.tw 告知

     ，以免影響他人權益。

七、 本組將各別發mail通知自費季節性流感疫苗的接種活動相關注意事項，

     It is time for seasonal flu vaccination again.

In order to provide vaccination shots in campus,

our division coordinated with Yeezen General,

which they agreed to provide seasonal flu vaccine shots in campus.

(If the number of registration is less than 50 people,

then the event will be canceled;

event is not applicable for children under 12 y/o）

1. Registration date: From 2015/10/12 to 11/02 17:00

2. Event time: 2015/11/04 (Wed) 09:00-13:30

3. Venue: Auditorium

4. Fee: Seasonal flu vaccine (VAXIGRIP) NT 450 on your own expense

   ( pay on site)

5. Seasonal flu vaccination online registration, please go to the

   Academic Information System

   ->Other Services in Campus->Reservation-->Event Registration

   (If you are a family member,

   please call Division of Health Services 03-5731051,

   we will assist the registration process)

6. If you have to cancel due to other reasons, please call

   or email clinic@my.nthu.edu.tw to notify us,

   so as not to affect the rights of others.

7. We will notify by email regarding this seasonal flu vaccination

   event and other notices.
