新進人員一般體格檢查Q & A

請｢各用人單位｣在發送新進人員錄取通知時，務請告知須先行至勞動部認可之勞工體檢醫療機構進行一般體格檢查，並於報到時繳交體格檢查報告。

• 新進人員「到職前」為什麼一定要完成一般體格檢查?
  1. 依職業安全衛生法 第2條  勞工：指受僱從事工作獲致工資者。
  2. 因依照依職業安全衛生法中第20條規定，雇主於僱用勞工時，應施行體格檢查，勞工對於第一項之檢查，有接受之義務。
  3. 職業安全衛生法規範雇主與勞工雙方有關勞工體格檢查之義務，目的係為識別勞工工作適性，評估其是否適合從事該作業，避免因工作造成勞工健康之威脅或傷害，因此，體格檢查應於勞工實際從事作業前完成，方能符合該目的。

• 新進人員一般體格檢查必須於何時完成呢?

新進人員「報到前」需自行至勞動部會商衛生福利部認可之勞工體格與健康檢查醫療機構進行一般體格檢查，並持完成之「報告正本」，於報到當日繳交至衛保組(各醫院體檢作業皆須7-30個工作日，敬請提早作業時間，以免影響當日報到完成之權益)。

• 未依規繳交一般體格檢查報告，會受罰嗎?

1. 會因違反職業安全衛生法第20條，處新臺幣三千元以下罰鍰。

• 新進人員一般體格檢查的項目包括那些?

1. 作業經歷、既往病史、生活習慣及自覺症狀之調查。
2. 身高、體重、腰圍、視力、辨色力、聽力、血壓及身體各系統或部位之理學檢查。
3. 胸部Ｘ光（大片）攝影檢查。
4. 尿蛋白及尿潛血之檢查。
5. 血色素及白血球數檢查。
6. 血糖、血清丙胺酸轉胺酶(ALT)、肌酸酐(creatinine)、膽固醇、三酸甘油酯、高密度脂蛋白膽固醇之檢查。
7. 其他經中央主管機關指定之檢查。

• 我要如何知道那些醫療院所，能夠完成新進人員一般體檢檢查?

1. 請至https://hrpts.osha.gov.tw/asshp/hrpm1055.aspx，查詢勞動部會商衛生福利部認可之勞工體格與健康檢查醫療機構，方可完成新進人員一般體格檢查。

• 本校認可並收取多久之內的新進人員一般體格檢查報告?
• 有效年限內之體格檢查報告並符合體格檢查報告項目為

1. 年滿65歲以上者，一年健檢報告。

2. 40歲以上未滿65歲者，三年內的健檢報告。

3. 未滿40歲者，五年內健檢報告。

.   校外學生身分於本校擔任兼任老師或是兼任助理，可依上述年限內繳交原學校進行之新生健檢報告。

.   每學期聘任之兼任老師繳交的體格檢查報告尚在規定年限之內時，則不須每次聘任每次繳交。

• 新進人員一般體格檢查費用由誰支付?

1. 職安法規定新進人員必須於「到職前」完成體格檢查，故本校新進人員一般體格檢查費用，需由新進人員自行支付費用。

• 學校會統一在校辦理新進人員體格檢查嗎?

1. 本校新進人員「報到前」，需「自行自費」至勞動部會商衛生福利部認可之勞工體格與健康檢查醫療機構進行一般體格檢查，並持完成之報告正本於報到當日繳交至衛保組(各醫院體檢作業皆須7-30個工作日，敬請提早作業時間，以免影響當日報到完成之權益)故學校「不會」統一在校辦理新進人員健康檢查。

• 學校在校辦理在職勞工一般健康檢查時，新進人員可以「自費」一起進行體檢嗎?

1. 職安法規定新進人員必須於「報到前」完成一般體格檢查，且在校辦理之在職勞工一般健康檢查，屬雇主付費，故新進人員「無法」於在職勞工一般健康檢查現場，自費接受新進人員體格檢查。

New Staff Health Examination Q & A

• Why is it necessary for new staff to complete the health examination prior to their first day of work?

1. According to Article 20 of the Occupational Safety and Health Act, the employers shall conduct pre-employment physical examinations for laborers at the time of employment. The laborers are obligated to accept the examinations.
2. By the Occupational Safety and Health Act, it is mandatory that the employee take the health examination to determine their suitability for the job and to prevent work-related threats or injuries. Thus, the health examination must be completed before the date of employment. 

• When must the new staff health examination be completed?

Prior to the first day of work, new staff are required to complete the health examination at a medical center approved by the Ministry of Health and Welfare. The original copy of the health report should be submitted to Division of Health Service on the date of employment (the health examination process for medical centers take from 7-30 working days, so it is recommended to complete the health exam in advance). 

• Will failure to submit the health examination report result in a civil penalty?
  1. Violation of Article 20 of the Occupational Safety and Health Act will result in a fine of no more than NT$3000.

• What are the inspection categories for the new staff health examination?

1. Operation experience examination, past disease history, self-awareness symptoms.
2. Height, weight, waistline measurement, eyesight, color identification, hearing, blood pressure, and physical examinations.
3. Chest X-ray.
4. Urine Protein and Occult Blood.
5. Hemoglobin and white blood cell count.
6. Blood sugar, ALT, creatinine, cholesterol, triglyceride, and HDL-C.
7. Other necessary examinations.

• How do I know which medical centers I can go to complete my new staff health examination?

to search for medical centers approved by the Ministry of Health and Welfare to complete your new staff health exam.

• What is the time frame for the new staff health examination report that NTHU is willing to accept?
  1. NTHU accepts health exam reports that are within 3 months before the date of employment. The inspection categories must include all the categories from NTHU’s new staff health exam form. New staff health exam inspection categories and forms can be found on the website of the Office of Personnel.

• Who should pay for the new staff health examination fee?
  1. According to the Occupational Safety and Health Act, new staff must complete the health exam prior to the date of employment. Thus, the new staff health exam fee should be paid for by the new staff themselves.

• Will NTHU host a new staff health examination on campus?
  1. According to part a) of Question 2 -> NTHU won't host a new staff health examination on campus.

• Can new staff take the employee health exam that NTHU hosts on campus at their own expense?
  1. According to the Occupational Safety and Health Act, new staff must complete the health exam before the date of employment. Also, the on campus employee health examination is paid for by the employer. Thus, new staff cannot complete the new staff health exam at their own expense during the on campus employee health examination.
